It allows us to drill down to root causes\, establish causal relationships\, and generate creative solutions that solve problems continuously. More robust tools like the 12-step RCA are typically used for high-risk problems. Ideally\, RCAs are multi-departmental activities\, so the involvement of stakeholders affected by the problem is essential to ensure that all perspectives are considered. \n12-Step Theory and Work Scenarios \n– Advantages of structured problem solving\n– Human biases and barriers to successful problem solving\n– Risk ranking and problem prioritization\n– RCA2Go software training\n– Problem definition\n– Focus phase: Exploration of the first four steps around a worked scenario\n– Find Causes: Develop understanding of the problem\, brainstorm for causes (Ishikawa / Fishbone) & develop cause tree.\n– Using the three-way test to validate causes\n– Exploration of Human Factors\n– Worked scenario from step 1 through to actually solving the problem\n– Fix Forever: Structured exploration of solution options through to completion\n– Worked scenario around Combination Causes \nWorkshop Methodology \nThis course is presented in a group setting with the participants involved in team-based activities.
